Addendum: I don't think 42. ... h4 is handy, on account of 43. Rd8 h3 44. Rd1 Ra6 (? for example) 45. Rh1 Nf4 46. c4, and White is the one playing for the win and not Black. 42. ... Ra6 43. Re2 does not worry Black as he can activate his Knight while the pawn is steadily waiting on h5 to come in action at a more appropriate moment.
Looks like you're right. Thought that delaying h3 (by 42... h4 43. Rd8 Ra6) would help, but he can force it (or Nf4) by 44. Rd2. So, I agree that 42...Ra6 is the way to go here.

Rd8-d1-h1 and now our pawn is held up indefinitely especially if its on h3 because then the knight is stuck on f4 defending it and can't chase the rook away.
No, we only need to maneuver our rook and knight if white decides to stop our h-pawn by sending his rook down. But if he does that, he is not moving his c-pawn so we don't have to worry about that.

If we push the h-pawn, white can try to stop it with his rook or he can race his c-pawn. If he tries to stop our pawn with his rook, our rook and knight should be able to protect our h-pawn. If he goes for the c-pawn promotion, we promote first with one extra move which we can use to check his king with our new queen until the queen can bag his c-pawn.

42... h4 43.Rd8 h3 44.Rd1 Nf4 45.Rh1... we can't stop this except by 42...Ra6 if, for example, we try the maneuver on move 43 after Rd8 it is too slow.

I'm probably missing something, but where can the rook get to from a6?

I'm not sure of a specific square. We gain access to a few new options for the rook. One possibility may be Re6 to force the trade of rooks. That doesn't necessarily do anything though. I think the main point is that it has a lot more options that it did on a5.

Its a prophylactic move. It seems our last couple moves have been a battle to keep whites rook out of h1 have they not? 41...Ng2 to prevent Re1-h1 so white now has to play Rd8-d1 first but that fails to ...Re5-e1 so white plays 42.Nb5 to prevent that, so to keep it going the next logical move is 42...Ra6.

I vote h4 !
We can play : h4, h3, and Nf4
The protected pawn on h3 will be big threat.
And after that we can activate a rook : Ra6.
Rook is activated and help for h2, h1-Q.

42...h4? 43.Rd8! h3 44.Rd1 Nf4 45.Rh1 is previously mentioned as the problem to 42...h4? by tom and heinz.
